Let us understand what is egg donation. For getting a pregnancy through assisted reproduction, we basically need three essentials. They are the egg (female seed), sperm (male seed) and the uterus (womb). Think of this as a plant which has to be grown in the soil. We would need to first prepare the soil, put good quality seeds and then after watering wait for the plant to grow. Similarly, the plant seed in this case is the sperm and the egg. The soil is the uterus. So it is obvious that if there are no female seeds (eggs) we would need them from somewhere. Egg donation programme is a type of ART treatment wherein instead of the patient’s eggs being fertilised by the husband’s sperm, it is donor eggs from a young healthy woman who agrees to donate her eggs to the infertile couple. The resulting embryo(s) is then transferred in the patient’s uterus to make her pregnant. She then delivers a child just like in normal pregnancy and feeds the baby like her own child. The only thing different in this case is that genetically, the baby belongs to the father (husband) and the anonymous young woman who donated her eggs.

Emotional and confidentiality aspects of Egg Donation

What are the Indications for Donor Egg IVF (who should go for egg donation treatment)?

  • To start with, patients who have no eggs left in their ovaries. This can happen after natural menopause, or it can happen prematurely, called us “Premature ovarian failure”
  • Very low ovarian reserve - this is known after checking the Antral follicles through ultrasound and/or doing a blood test called AMH
  • Surgical removal of ovaries
  • Women with advanced age or in the peri-menopausal group
  • If you have received chemotherapy or radiotherapy that damages ovaries
  • Patients with known genetic disease
  • Rarely congenital conditions like gonadal dysgenesis wherein the patient doesn’t have any eggs from birth

Matching the right egg donor for you

Once we have decided that indeed egg donation is the right treatment for you, we begin the process of matching you with the most appropriate egg donor. We would take your picture and send it to a third party Agency. The Third Party Agency would match your details with their large database and select the most appropriate donor for you. Characteristics noted are your appearance, eye colour, hair colour, skin colour, height, weight and blood group/type. We pay due diligence to all your preferences like background, religion and educational status of the egg donor and those details about the donor will be shared with you. What will not be shared with you, however, is the personal details of the donor like her name, address and identification. This is because we at Janini IVF, providing the best egg donation Treatment in Delhi NCR, stringently follow the anonymous donor program in strict accordance with the ICMR guidelines. Please note that the donor would also not be knowing where her eggs are going (the donor would not know about your identity).

Documentation

Our egg donation program is in accordance with HFEA (Human Fertilisation and embryology authority), ASRM (American Society of Reproductive medicine) and ICMR (Indian Council of Medical Research) guidelines. You will both (husband and wife) be asked to sign consent forms for availing egg donation treatment. It is mandatory for both partners to sign on the forms and our forms are also in accordance with the ICMR guidelines. Some patients come up with an egg donor who is a family member or relative or friend. This is not acceptable as it is mandatory to have only an anonymous donor, as per the ICMR norms.

Recruitment of the selected donor

Once we have matched you with the right donor, we begin the recruitment process in which she is made to undergo an extensive panel of investigations. These include three basic tenets of testing:

  1. We check the donor’s egg reserve by doing Antral follicle scan and AMH blood test. We ensure that the donor will have a good oocyte yield both in terms of quantity and quality.
  2. Infection screen - the donor undergoes blood tests like VDRL, HIV, Hepatitis B virus, Hepatitis C virus
  3. Genetic screen - we rule out hereditary diseases in the donor as we are making the embryos out of her eggs. Some conditions tested for include Thalassemia, G6PD deficiency, Sickle cell anaemia, etc.

Only after these tests is a donor recruited as a donor for you. Dr Dalal personally oversees the selection process of the donor. The recruitment process also includes a thorough history of the donor, family history, personal and obstetric history, medical history and education status. The entire process of matching and recruitment does not take more than a few days.

Synchronisation of donor-recipient and ovarian stimulation of the donor

Once a donor is confirmed for you, her cycles are synchronised with yours in such a way that her oocyte pick-up (egg retrieval) procedure will coincide with your endometrium being ready for embryo transfer. The donor is given daily gonadotropin injections and you are given oestrogen pills to prepare your endometrium. At the time of oocyte pick-up of the donor your husband will be asked to give his semen sample. We would also have a frozen back-up sample of his so that if there is any problem in him giving a sample on the day of pick-up there is no issue.

  • After the embryos are made, you will be informed about when to come for embryo transfer. On the day of transfer complete clarity will be given to you about the number and quality of embryos formed. In our state-of-the-art facility of Janini IVF usually you will have enough top quality embryos from egg donation. We generally transfer not more than 2 or 3 embryos embryos and freeze the rest so that you have extra embryos frozen as well. After embryo transfer there will be a pregnancy test done after 15 days and with egg donation treatment our success rates are close to 70-80% clinical pregnancy rates per attempt.

Frequently asked questions :

As per ICMR guidelines, the egg donor program has to be anonymous. The donor is not aware about the fate of her eggs just as how you would not be told about the identity of the donor. All her features like colour, blood group, education status, etc will be revealed to you though.
No we don’t encourage this. All the eggs found from one donor would be allocated to you and any extra embryos formed thereof would be frozen safely. This way you can have advantage of generated many more embryos from a single treatment attempt.
As mentioned, there are 3 tenets of testing the donor - (i) Ovarian reserve testing (ii) Genetic testing to rule our hereditary diseases (iii) Infectious disease screening for diseases like HIV, HBsAg, Hepatitis C, etc.
Donors are usually married and already have at least a single child of their own. They are aged between 21-29 years of age.
There is no limit to the age and you can get pregnant even after your menopause with egg donation as the eggs are coming from elsewhere and all we need is your uterus. However if you are above 50 years of age we will do medical fitness to see if you can carry a pregnancy till term.
You will be definitely told about all characteristics of the donor. If you have personal preferences with respect to religion, education, caste, etc. we can look into it to choose the best donor for you.
No, as per the ICMR guidelines we have to follow the anonymous donor protocol
Around 12-16. But not all of them would be M2 or mature eggs, some would be immature.
Usually 2-3 weeks, but in case you are an International client or cannot stay in Delhi for that long, we do have a premium program in which we can allocate a pre-screened donor to you.
